Submitting a photo will give us a more complete picture of the student

Bank statement with funds in USD must show enough funds to cover a full academic school year

☐ Pay School Application fee

Step 2: Admissions Decision

☐ Receive admission decision within 7 days after school receives complete application materials including payment

Step 3: Enrollment and I-20 Application

☐ School Contracts and enrollment forms

☐ I-20 Application

☐ Pay Registration and I-20 fees
Pay fees required to enroll and receive the Form I-20 (registration, tuition deposit, I-20 application fee)

Step 4: Issue Form I-20

☐ Receive Form I-20 within 7 days after school receives complete registration and I-20 application materials including payments

Step 5: Prior to first day

☐ Immunization Records 
Records must show that the student meets state immunization requirements. Must be official, stamped, signed and dated by the physician

☐ Physical Exam
Must have been completed within the past year.
